ABRAMSON SENIOR CARE OF JFCS
As of October 1, 2025, Jewish Family and Children’s Service of Greater Philadelphia (JFCS) has merged with Abramson Senior Care in order to create a single, comprehensive resource for older adults and their families, known as Abramson Senior Care of JFCS.

Abramson Senior Care of JFCS services include:
- Care Management: Through a team of licensed social workers, care managers, and certified registered nurse practitioners (in partnership with Suburban Geriatrics at Home, P.C.), our care management services include collaboration with a patient’s primary and specialty physicians to support treatment plans, coordination of healthcare, and extensive caregiver education.
- Chronic Care Management: This continuous approach helps seniors better manage chronic conditions, stay on track with their health goals, and receive the guidance they need between appointments by bridging the gap between office visits through ongoing check-ins from a dedicated care team, personalized care planning, and coordinated support.
- GUIDE (Guiding an Improved Dementia Experience) Model: This community-based model allows seniors and family caregivers to receive specialized support at home by providing comprehensive care management for individuals with a dementia diagnosis who have Medicare as their primary insurance.
