GUIDE is a true lifeline, providing comprehensive care management for seniors with a dementia diagnosis who have Medicare as their primary insurance.
GUIDE can transform quality of life for individuals and families living with dementia and allow seniors and family caregivers to receive specialized support at home.
These invaluable resources can help your family, including:
- Safety in the Home: Assessment and plans to avoid injuries in the home.
- Assistance with ADLs (Activities of Daily Living): Coordinating care to help with the many ways we take care of ourselves each day, such as eating independently, dressing, bathing, and using the facilities.
- Assistance with IADLs (Instrumental Activities of Daily Living): Facilitating care to help with complex tasks such as managing finances, shopping, medication dosages, housekeeping, cooking or food prep, transportation, laundry, and shopping.
- Coordination of Care Plan Services: Helping you with healthcare appointments, meals assistance, area resources, and more.
- Caregiver Support: GUIDE also focuses on the unpaid family members who care for their loved ones suffering from a dementia diagnosis. Get resources, education, advice, and other tools to manage stress and build caregiver skills. We can help you with maintaining emotional goals, cognitive wellness, socialization, respite support, and connection to programs.
- Managing Behaviors: A caregiver may also look for resources to manage specific issues with their loved one. Abramson provides a strengths-based perspective to support families and their custom care plans throughout the dementia care process.
